Rhode Island School Physical Exam Form:
All students who are entering school in Rhode Island for the first time are required to have a physical exam. All students entering 7th and 12th grades are also required to have a physical exam. Please send the completed physical exam form to your child's school or have it faxed to the school nurse. The Rhode Island physical form can be downloaded here.

School Immunization Requirements:
In accordance with the Rhode Island Department of Health Rules and Regulations Pertaining to Immunization and Testing for Communicable Diseases (R23-1-IMM), the minimum number of doses of required immunizations for children entering school is as follows:

Immunization Requirements for Kindergarten Entry:
- Three (3) doses of hepatitis B Vaccine
- Five (5) doses of DTaP (diphtheria, tetanus, pertussis) vaccine
- Four (4) doses of Polio vaccine (last dose after four years of age)
- Two (2) doses of MMR (measles, mumps, rubella) vaccine
- Two (2) doses of varicella (chickenpox) vaccine or a statement signed by your child's doctor stating that your child has a history of chickenpox disease.
Immunization Requirements for 7th Grade Entry:
- Booster dose Tdap (tetanus, diphtheria, pertussis) vaccine, if it has been 5 years or more since the last dose of diphtheria-tetanus containing vaccine.
- Four (4) doses of Polio vaccine
- Two (2) doses of MMR vaccine (Measles, Mumps, Rubella)
- Three doses (3) doses of hepatitis B vaccine
- Two (2) doses of varicella (chickenpox) vaccine or a statement signed by your child's doctor stating that your child has a history of chickenpox disease
- One (1) dose of Meningococcal conjugate (Meningitis) vaccine
- One dose of HPV (Human papillomavirus) vaccine

- Serves uninsured children and insured children who do not see one doctor or healthcare provider reugularly for their primary healthcare needs.
- Adults without health insurance (uninsured) who need vaccines to attend school or work.
- Adults with health insurance who are unable to pay all or some costs for vaccines for school or work (underinsured).

Vaccinate Before You Graduate Program
High Schools Vaccinate Before You Graduate (VBYG) is a program that provides vaccinations to all students in grades 9 through 12 in Rhode Island schools. The following vaccines are available to students through VBYG: hepatitis A, hepatitis B, human papillomavirus (HPV), flu, MMR (measles, mumps, and rubella), meningitis, polio, Tdap (tetanus, diphtheria, and pertussis), and varicella. Please contact your child's school nurse if you have any questions.
Screening Requirements:
The State of Rhode Island mandates the following screenings to be completed in school:
Dental: screening is done by a licensed dentist or hygienist yearly in grades K through 5 and grade 7.
Vision: screening is done upon school entry (except for students who have documentation of being screened by their own doctor), grades 1, 2, 3, 4, 5, 7, and 9.
Hearing: screenings are done by the RI School for the Deaf in grades K, 1, 2, 3, Special Education and referrals.
Scoliosis: screenings are done in grades 6, 7, and 8.

Pawtucket School Department Medical-Related Forms:
Over The Counter Medication Form allows the nurse to medicate your child with tylenol, ibuprofen, an antacid, or benadryl as needed. This form needs to be completed by a parent yearly and returned to your child's school in order to allow the nurse to medicate your child with over-the-counter medication. Medication Request Form needs to be completed by a physician and signed by a parent for a student to receive daily medication in school. This form needs to be completed yearly and returned to your child's school in order to allow the nurse to medicate your child with a prescribed medication.
